You can now enroll online in our patient care platform on Oakhaven.Alas.MD. Â Once you enroll, we will give you instructions about how to get scheduled for your first visit.
What does enrolling mean?
To enroll means that you provide your basic contact information and subscribe as a member. See the pricing page for details about membership costs for age groups.
For Dr. Jordan's patients, subscription fees will be delayed until her start date on April 15 when you will be charged for April pro-rata (for just the second half of the month).
The last page of enrollment requests a credit card. If you intend to use an ACH to pay for your subscription, send an email to admin@oakhv.com for an assisted pre-enrollment. You can also email if you have any other questions about subscriptions or billing.
Oakhaven Health is not signed with any insurance companies. Oakhaven is opted out of Medicare, Medicaid, and has no network contracts with any insurance companies. While DPC fees can be reimbursed with HSA funds, subscription costs do not count toward health plan deductibles and are not reimbursable out of network. By enrolling you agree that you will not turn in your membership fees to your insurance company and that you understand that Medicare and Medicaid will not reimburse for services onsite at Oakhaven Health DPC.

iPad users will need to allow session storage in Safari or use Google Chrome browser to complete the enrollment form.
Company Field
The field on the enrollment for company can be left blank. It is intended to be for your company, if your company is paying for your DPC membership directly. At this time Oakhaven does not have arrangements for employer-based plans. There is a an area for health insurance information on a following page that is for us to send with your lab orders and prescriptions that are sent out of the Oakhaven office.
SHORT FORM (with data transfer)
If you have access to your previous provider's patient portal and you can download CDDA XML file, our technical staff can walk you through downloading your PHR data and uploading it through a secure transfer utility to get it into Oakhaven's system. When you get into the enrollment form on Atlas.MD look for the button that says "Save and proceed to next step". Clicking that will let you complete the short form since you will be downloading your portal records and uploading them through our PHR Bridge.
Once you complete your enrollment, you can get a headstart gathering your data from your previous patient portal following the instructions at oakhaven.healthware.dev

LONG FORM
If you are unable to download from your previous patient portal, you should also use the long form or provide that additional information at your initial visit At the bottom of the first page you should click "Add more details" to continue providing more in-depth demographics and medical information.
